Notice of Privacy Practices

Premier Counseling Services, LLC creates a record of services you receive from us. We call this record your health information. We are required by federal law, the Health Insurance Portability and Accountability Act of 1996 (HIPPA), to keep your health information private.

You will be notified if a breach of your protected health information occurs through any preventable act if performed by any representative of Premier Counseling Services, LLC.

In Case of Emergency.

We may either use or release your health information in an emergency treatment situation.

To Obtain Payment for Services/Treatment Rendered.

We will use and disclose your health information to bill and collect payment for services/treatment provided by Premier Counseling Services, LLC. For example, your health information will be sent to your insurance company if necessary in order to receive payment for services rendered.

In Attempt to Avert a Serious Threat to Health or Safety.

We may release your protected health information when necessary in an attempt to prevent a serious threat to your health and safety or the health and safety of the public or another person. For example: In cases of child and/or elderly abuse or neglect.
